Apply for a Grant

We have developed a two-phase approach to make the review process more effective and efficient.

This approach lets us determine which programs are the best matches for our investment strategies and preferred areas of focus. If you are interested in receiving funding from the Foundation, your first step is to submit the Letter of Concept form. We will review it carefully. If we decide your organization is a good match, you will be invited to submit a full application.

Letter of Concept

We will not accept incomplete Letter of Concept forms.

The form will ask for this information:

  1. Contact information
  2. Project title
  3. Total amount of grant request
  4. Length of grant 
  5. Counties served by the project
  6. Description of project including:
    • Health needs of the population served
    • Anticipated results
    • Expected timeframe of project
    • How the funds will be used

Access to the Fall 2017 Letter of Concept will be available July 1, 2017. 

Deadline for Submission

You must submit the Letter of Concept for the Fall Grant Cycle by 5 p.m. on August 16,  2017.
